Payment for services is required upfront, based on the rates we have outlined on our website or provided during scheduling.
2. Superbill for insurance submission
After each sesion and if requested, we will provide you with a superbill. This document includes all of the information for your insurance company needs to process a claim such as: diagnosis codes, service details, and provider credentials.
3. Reimbursement from insurance
You can submit the superbill to your insurance company for potential reimbursement. If your insurance plan includes out-of-network benefits, your insurance company may reimburse you directly for a portion of the cost.
4. No impact on our fees
Your reimbursement does not affect the payment you made to us. Once payment is made to Live Meaningfully OT, it is your responsibility to work with your insurance provider to recover eligible funds.
